  • All you need is a valid Oklahoma ID or Driver’s License. We can use a separate mailing address if your address isn't current. If you have medicare, soonercare, or are a 100% disabled veteran— please bring proof of that. (i.e. card, screenshot, or letter from VA)

  • We take walk-ins every day we are open.

    Tuesday 9am to 5pm, we stop seeing patients at 4.

    Friday 9am to 5pm, we stop seeing patients at 4.

    Saturday 9am to 12pm, we stop seeing patients at 11.

    We ask that you arrive at least one hour prior to closing to give us enough time to see you and process your application.

Upon arrival you will be given new patient paperwork. The doctor will then discuss your health history with you and determine whether you are a candidate for medical marijuana. (Which you are, everyone is!)

Oklahoma does not require any qualifying conditions to apply for your medical card.

Your appointment with the doctor will take no longer than 15 minutes and you should plan to spend 30-45 minutes at our office if you plan to have our staff assist you with the registration process.

Come Prepared

You must be an Oklahoma resident to apply for the two-year medical marijuana license. We will ask you for the following information upon arrival:

  • Driver’s license or state issued I.D.

    • If your driver’s license does not reflect your current physical address, please mention this to our staff.

  • Patient’s full legal name

  • Residence and mailing address

  • Date of birth

  • Telephone number and email address

  • Brief health history

Apply Online

We are here to help you every step of the way.

Applications and state fees are collected by OMMA through an online registration system. We will create an account for you that you will need access to for future renewals and updates. Please have the following for this portion of the process:

  • Proof of residency (this can be any government issued photo I.D. You DO NOT need to bring a utility bill or lease agreement)

  • Proof of identity

  • If applicable, Medicaid/Medicare, Sooner Care enrollment documentation

  • If applicable, letter from the VA stating 100% disability

This form must be uploaded to OMMA’s online database within 30 days of the date signed by our physician.

 

FAQs

Do I need any records from my primary care physician?

You do not. Oklahoma does not require any preexisting conditions to apply for your license. Our doctor will, however, ask you what symptoms you will be treating with cannabis.

How long does it take to receive my card?

After submitting your application online, you should receive email confirmations as your application makes its way through the process. Typically patients who are approved will get their card in the mail within 14-18 business days.

How much does a patient recommendation cost?

The standard cost of the license is $104.30. This fee charged when you apply with your signed recommendation form online. Sooner care or medicaid patients and veterans who are 100% disabled pay a discounted fee of $22.50.

How do I renew my license?

You will see the doctor just as you did for your original license. Same form, same state fees. We will need a copy of your drivers license or state issued I.D. You do not need an appointment. Visits typically take less than 30 minutes.

What happens after I apply?

Once you have seen our doctor and received your signed recommendation form, we will create an account on OMMA’s website and upload all of your documentation. State fees are accepted through the OMMA’s website so a debit/credit card is required.

How long is my license good for?

Oklahoma medical marijuana licenses are good for two years after the date it was issued which is listed on the front of your medical card.
